– Confirm blood pressure reading
– Recheck using manual cuff if possible
– Assess for neurological or cardiac symptoms
Positioning & environment:
– Keep patient in a comfortable, semi-Fowler’s position
– Maintain a calm, quiet environment to reduce stress
Interventions:
– Administer prescribed antihypertensive medication (if available and ordered)
– Provide oxygen if patient is breathless
– Monitor vital signs closely (every 5–10 minutes)
Communication:
– Notify physician urgently
– Call EMS if BP remains dangerously high or symptoms worsen
