Rapis Assessment: The ABCDE Approach

A – Airway
– Ensure airway is clear and open
– Look for obstruction, stridor, or difficulty speaking
B – Breathing
– Check respiratory rate, depth, and pattern
– Assess oxygen saturation (SpO₂)
– Provide oxygen if <94%

C – Circulation
– Check pulse, blood pressure, capillary refill
– Look for signs of shock (cold, clammy skin)
– Control bleeding if present
D – Disability
– Assess level of consciousness (AVPU: Alert, Voice, Pain, Unresponsive)
– Check pupil size and reaction

E – Exposure
– Expose patient as needed for full assessment
– Look for injuries, rashes, swelling
– Maintain warmth with blanket
